Many of the providers with whom we coordinate provide transport for children to and from visits. Use these resources to help you bill for that service as well as the visitation sessions.
The Family Time contract defines unusual incident as: “circumstances or events that concern a child’s health, safety or well-being or may impact the child’s well-being.” Here you will find requirements on reporting these incidents.
These forms can be used by Family Time Providers to log reports, transportation and staff training. Feel free to download and use as needed for your agency.
